What to know about water kefir
Water kefir is cultured with sugar-water grains (tibicos) instead of milk, and that single substitution is the entire point of the drink. It grades clean on lactose and clean on dairy proteins, so the two things that make milk kefir difficult for most people who react to kefir are simply absent. What does carry over is the fermentation itself. Water kefir grades moderate for histamine, mildly liberating, and moderate for tyramine, so if your kefir reaction is amine-driven rather than dairy-driven, switching to the water version will not fix it on its own. The sugar-water base also leaves moderate FODMAP and moderate excess fructose behind and grades poorly for SIBO, because you are drinking live cultures alongside residual fermentable sugar.
Watch for
- Clean on lactose and on dairy protein, which is why it is the standard answer for people whose problem with kefir is the milk rather than the culture
- Moderate histamine with a mild liberator flag: live lactic fermentation builds biogenic amines whether or not dairy is involved
- Moderate tyramine, relevant if you take an MAOI or if amine-heavy foods reliably give you headaches
- Moderate FODMAP and moderate excess fructose from sugar left over in the base, and a poor SIBO grade because live cultures plus fermentable sugar is exactly the combination that feeds bacterial overgrowth
- Second ferments flavoured with fruit juice add fructose and amines on top of the plain batch, so a bottled fruit-flavoured water kefir is a bigger dose than a home-brewed plain one
