What to know about milkshake

A milkshake is about the heaviest dairy load you can drink. Milk plus ice cream means very high lactose and very high dairy protein in one large, cold serving, and the sugar and syrup push it to a high overall FODMAP score that also makes it one of the harder items on this list for SIBO. The volume is the real problem: you take in far more lactose from a single cold shake than from milk on cereal, and you tend to drink it fast, which delivers the whole dose at once.
