About Milk

Milk carries two completely separate trigger stories, and most people blame the wrong one.

Lactose, the milk sugar, is graded high and is a digestive problem: without enough lactase the sugar travels on to the colon, ferments, and returns as bloating, wind, cramps and loose stools, strictly by dose. Milk protein, meaning casein and whey, is graded high too and is an immune problem: cow's milk protein allergy shows up as hives, vomiting, eczema flares or worse, and milk grades as incompatible with an eosinophilic esophagitis elimination diet. Fresh milk is otherwise graded clean on histamine, salicylates, gluten and nickel, so when a glass of milk leaves you unwell, the sugar or the protein is almost always the answer, and which one it is decides what you can safely drink instead.

What trips people up

  • Graded high in lactose and high FODMAP, so a full glass is a far bigger challenge than the splash in a coffee. Lactose intolerance is dose dependent, and most people who have it still handle small amounts taken alongside food.
  • Graded high in dairy protein. Casein and whey are the allergy half of the story, and they survive every process that targets the sugar, including lactase drops and lactose-free labelling.
  • Graded incompatible with an EoE elimination diet. Milk is the food most often implicated in eosinophilic esophagitis and the first one taken out.
  • Borderline for SIBO, where lactose that was never absorbed feeds bacteria that have crept too far up the small intestine
  • Graded clean on histamine, unlike aged cheese, kefir and other long-fermented dairy, so a flushing or headache reaction to fresh milk is unusual and worth questioning

Common questions about Milk

What is the difference between lactose intolerance and a milk allergy?

They involve different parts of the milk and different systems. Lactose intolerance is an enzyme shortage: you make too little lactase, the sugar reaches the colon undigested, and you get gut symptoms an hour or several hours later, in proportion to how much you drank. A milk allergy is your immune system targeting the milk proteins, so reactions can start within minutes, can involve skin and airway as well as gut, and can be serious at tiny doses. Doctors separate the two with different tests, a hydrogen breath test for the sugar and skin-prick or IgE blood testing for the protein.

Is milk high in histamine?

Fresh milk is not. It grades clean for histamine and is not a liberator, which surprises people who have been told to avoid all dairy on a low-histamine diet. Histamine climbs with time and fermentation, so aged cheese, kefir and long-cultured dairy are the real concerns while fresh milk is not. If milk itself reliably upsets you, lactose or the milk protein is the more likely mechanism.

How much lactose can I have if I am lactose intolerant?

More than most people expect. Research on lactose malabsorption consistently finds that around 12 g in one sitting, roughly a cup of milk, is tolerated by most adults with few or no symptoms, especially when it comes with a meal rather than on an empty stomach. Spreading dairy across the day works better than one large serving. Intolerance is a threshold, not a switch, and finding your own threshold is the point of logging it.

Is A2 milk easier to digest?

A2 milk comes from cows that produce only the A2 form of beta-casein rather than the more common A1 form. It has the same lactose as ordinary milk, so it does nothing for lactose intolerance, and it is still cow milk protein, so it is not safe with a milk allergy. Some people who feel unwell after regular milk do report less discomfort on A2, and there is research behind the idea, but it is still limited. Treat it as worth a trial, not as a dairy-free option.
