What to know about erythritol
Erythritol is the sugar alcohol that refuses to behave like the other sugar alcohols. It is graded high on polyols, because chemically that is exactly what it is, yet it sits at the clean end of the overall FODMAP scale, and that apparent contradiction is the whole story. Roughly 90 percent of what you swallow is absorbed in the small intestine and passed out unchanged in urine, so very little reaches the colon to be fermented. That is why low FODMAP bakers reach for erythritol when sorbitol, xylitol and maltitol leave them bloated, and why it is tolerated at doses that would make those three unusable. It is clean on histamine, gluten, lactose, salicylates, caffeine, sulfites and nickel. The genuinely open question about erythritol is not digestion at all: a 2023 Cleveland Clinic paper in Nature Medicine linked high circulating erythritol to heart attack and stroke and showed increased platelet clotting activity, and that debate is still live.
Watch for
- Graded high on polyols, so a strict low FODMAP elimination phase will still ask you to leave it out, even though its absorption profile is far friendlier than sorbitol, xylitol or maltitol
- Dose still decides it.Around 30 to 50 g in one go (a whole tray of sugar-free baking, or several servings of sugar-free sweets) can bring on nausea, rumbling and loose stools in people who handle a teaspoon or two without trouble
- Most tabletop stevia and monk fruit sweeteners are mostly erythritol by weight, with the intense sweetener present in traces.If a stevia blend upsets you, erythritol is the likelier culprit than the stevia
- The 2023 Nature Medicine finding is an association, not proof of cause.The body also makes small amounts of erythritol from glucose, so raised blood levels may partly mark existing metabolic disease. A 2024 follow-up did find platelet activation in healthy volunteers after a 30 g drink, which is why the question stays open
- Hides in sugar-free gum, protein bars, keto baking blends and zero sugar drinks, sometimes labelled only as E968 or as part of a sweetener blend
