About Pomelo

If you have been told to avoid grapefruit because of a medication, that warning covers pomelo, and hardly anyone is told so.

Pomelo is the largest citrus and the direct ancestor of grapefruit (grapefruit is a pomelo crossed with a sweet orange), and the trait it passed down is the furanocoumarin content that inhibits intestinal CYP3A4. There is a published case of a kidney transplant patient whose tacrolimus level climbed after eating slightly under 100g of pomelo, and animal work found the inhibitory effect from a single dose of pomelo juice still present three days later. On the food-chemistry side pomelo is actually the gentler fruit of the two: salicylates C against grapefruit D, oxalates C against grapefruit E, FODMAPs B against grapefruit D.

  • Furanocoumarins are the headline. Pomelo carries the CYP3A4-inhibiting compounds behind the grapefruit effect, which raise blood levels of drugs including some statins, tacrolimus, ciclosporin and certain calcium channel blockers. Because the enzyme has to be replaced rather than merely freed up, spacing the fruit away from your dose does not fix it. Raise it with a pharmacist rather than guessing.
  • Salicylates C and oxalates C. Borderline on both, but pomelo is eaten as fistfuls of large segments rather than in wedges, so a casual portion is far bigger than a portion of any other citrus.
  • FODMAPs B and excess fructose B. Better than grapefruit at D, though half a pomelo in one sitting is a substantial fructose load for a sensitive gut.
  • Nickel B and glutamates B: mild, but worth logging if either is on your list.
  • Peeling away the thick rind and the bitter membrane does not remove the interaction. The drug interaction work has been done on juice and pulp, which is exactly the part you eat.

Common questions about Pomelo

Does pomelo interact with medications the way grapefruit does?

Yes. Pomelo contains the same furanocoumarins, principally bergamottin and 6,7-dihydroxybergamottin, that disable CYP3A4 in the gut wall and let more of certain drugs into the bloodstream than the dose intended. It has been documented raising tacrolimus levels in a transplant patient, and in animal work the effect from one serving lasted around three days. Grapefruit warnings on a medicine leaflet should be read as covering pomelo even when it is not named.

Is pomelo safer than grapefruit?

It depends which risk you mean. On food-chemistry triggers pomelo is clearly milder: salicylates C rather than D, oxalates C rather than E, FODMAPs B rather than D. On the drug interaction they are in the same category, so a person who switched from grapefruit to pomelo to protect their gut may have kept the more serious of the two problems.

Is pomelo low FODMAP?

It grades B for FODMAPs and B for excess fructose, which puts it in the moderate, portion-dependent band rather than the safe or the high one. It is a calmer choice than grapefruit (FODMAP D) if citrus is on your list, but pomelo segments are enormous and people eat several without counting, so weigh the portion rather than counting pieces.

Which citrus fruits do not have the grapefruit interaction?

Sweet oranges, mandarins and clementines, and kumquats are not sources of the furanocoumarins involved, and when Florida tangelo varieties were tested almost all contained none at all. The citrus that do carry them are grapefruit, pomelo and Seville (bitter) orange, plus limes at lower levels. This is about the specific compounds rather than about citrus or acidity in general.
