About Water kefir
Water kefir is cultured with sugar-water grains (tibicos) instead of milk, and that single substitution is the entire point of the drink.
It grades clean on lactose and clean on dairy proteins, so the two things that make milk kefir difficult for most people who react to kefir are simply absent. What does carry over is the fermentation itself. Water kefir grades moderate for histamine, mildly liberating, and moderate for tyramine, so if your kefir reaction is amine-driven rather than dairy-driven, switching to the water version will not fix it on its own. The sugar-water base also leaves moderate FODMAP and moderate excess fructose behind and grades poorly for SIBO, because you are drinking live cultures alongside residual fermentable sugar.

- Moderate histamine with a mild liberator flag: live lactic fermentation builds biogenic amines whether or not dairy is involved
- Moderate tyramine, relevant if you take an MAOI or if amine-heavy foods reliably give you headaches
- Moderate FODMAP and moderate excess fructose from sugar left over in the base, and a poor SIBO grade because live cultures plus fermentable sugar is exactly the combination that feeds bacterial overgrowth
- Second ferments flavoured with fruit juice add fructose and amines on top of the plain batch, so a bottled fruit-flavoured water kefir is a bigger dose than a home-brewed plain one

Common questions about Water kefir
Does water kefir contain lactose or dairy?
No. Water kefir grades clean on both lactose and dairy proteins. The grains are cultured in sugar water rather than milk, so there is no casein, no whey and no lactose in a standard batch. The one thing to check is the maker: a few producers rest their grains in milk between batches, which is unusual but worth asking about if you are severely dairy allergic rather than lactose intolerant.
Is water kefir safe if you have histamine intolerance?
Only cautiously. Water kefir grades moderate for histamine and mildly liberating, plus moderate for tyramine. That is lower than milk kefir, which grades high on histamine and high on tyramine, but moderate is not zero. If you are testing it, start with a small glass of a young plain batch rather than a long-fermented or fruit-flavoured bottle, and log it against your usual symptoms.
Is water kefir low FODMAP?
Not reliably. It grades moderate for total FODMAP and moderate for excess fructose because fermentation never consumes all of the sugar it was fed. It is also graded poorly for SIBO. A small glass may sit fine, but treating it as a free food the way you might treat plain water or black tea is a common way to end up bloated without knowing why.
Water kefir or milk kefir: which should I try if kefir upsets me?
It depends which part of milk kefir is the problem. On our grades milk kefir is high for lactose, high for dairy protein, high for histamine and high for tyramine. Water kefir clears the first two entirely and drops the amine grades to moderate. So a lactose or casein reaction should improve a lot on the water version, while an amine or histamine reaction may only improve partly.
