Tyramine and migraines: the aged-food connection

Why aged cheese and cured meat are the usual migraine suspects - and what the science actually says.

Every person with migraine eventually hears the warning: watch the aged cheese, the red wine, the cured meats. And somewhere in that advice, one word keeps surfacing as the culprit - tyramine.

It's a real compound with a real history. But the story around it is a mix of solid chemistry and inherited folklore, and it's worth separating the two. So here's tyramine properly: what it is, why it concentrates in aged foods, what the evidence genuinely supports, and how to find out whether it matters for you.

What tyramine actually is

Tyramine is a trace amine - a small molecule found naturally in foods, plants and animals.1 It belongs to the same broad family as histamine: the biogenic amines. And like histamine, it's "vasoactive" - it acts on blood vessels and the nervous system. That property is the entire reason it ever got linked to migraine.

It isn't a contaminant or an additive. It's a normal product of normal food chemistry - which is exactly why it's impossible to avoid completely, and why the useful question is about amount, not presence.

The aged-food connection, explained

Here's the part that makes "tyramine" and "aged food" almost synonymous.

Tyramine is built from an amino acid called tyrosine. As food ferments, ages, cures or simply spoils, bacteria carrying a particular enzyme convert that tyrosine into tyramine.1 The more microbial activity and the more time, the more tyramine accumulates.

That single mechanism explains the entire "suspect list." Tyramine is high in:1

  • Aged cheeses - cheddar, blue cheese, feta and other matured varieties
  • Cured and salt-dried meats - salami, aged sausage, cured fish
  • Fermented vegetables - sauerkraut, kimchi
  • Fermented soy products - soy sauce and similar
  • Wine and beer
  • Some produce eaten overripe, and foods left to sit

The pattern is consistent: age it, ferment it, cure it, or let it linger, and tyramine climbs. Fresh, freshly-cooked food is generally low. (For the full breakdown, see tyramine foods.)

A counterintuitive twist: modern food has less tyramine

Here's something the folklore misses. Modern improvements in food storage and handling - better temperature control, controlled fermentation cultures, faster supply chains - actually reduce tyramine in commercial foods.1

The classic, dramatic "cheese reaction" that built tyramine's fearsome reputation in the mid-twentieth century is, partly for that reason, rarer now. The same-named cheese on a modern shelf may carry considerably less tyramine than its equivalent decades ago. It's a useful reminder that "aged cheese" is not a fixed dose - it's a range, and freshness and handling move it.

Why tyramine could plausibly trigger a migraine

To see why tyramine became a migraine suspect, you need its mechanism.

Tyramine is an indirect sympathomimetic. Once absorbed, it prompts your nerve endings to release stored neurotransmitters - most importantly noradrenaline - which drives vasoconstriction, a faster heart rate and a rise in blood pressure.1 In other words, tyramine nudges the body's "fight or flight" chemistry.

Migraine, meanwhile, is a neurological event in an already-excitable brain - a brain that is more easily tipped into an attack. A compound that acts on blood vessels and adrenergic signalling is, on the face of it, a plausible nudge toward that edge.1 That plausibility - not proof - is what tyramine has always had.

The strongest evidence is a drug interaction

There is one place tyramine's effect on the body is beyond doubt - and it's worth understanding, because it's both the source of tyramine's reputation and the reason that reputation is often overstated.

Tyramine is normally broken down by an enzyme called monoamine oxidase (MAO).1 MAO is the safety valve - it clears tyramine before it can do much. But a class of older medications, MAO inhibitors, switch that valve off. In someone taking an MAOI, a tyramine-rich meal isn't cleared - and a large enough dose can drive a dangerous spike in blood pressure, complete with severe headache. This is the famous "cheese effect."1

It's genuinely important context - it proves dietary tyramine can have real, dose-dependent physiological effects. But notice what it actually shows: a strong reaction occurs when a drug has removed the body's tyramine defence. Extrapolating from that to "tyramine causes ordinary migraines in people not on those drugs" is a leap - and it's exactly the leap the evidence struggles to support.

The honest state of the evidence

So does tyramine trigger everyday migraine? The careful answer is: possibly, for some people - but the evidence is mixed and far from settled.

Tyramine is widely described as a potential migraine trigger, and the adrenergic mechanism above is a reasonable proposed route - but the research itself is hedged, and reviews note that the picture "requires additional study."1 A critical review of tyramine sensitivity in dietary migraine concluded that supportive studies are sparse and inconsistent, and that dietary tyramine alone is rarely, if ever, the major cause of an attack.2

That's not a reason to dismiss tyramine - it's a reason to treat it as a suspect to investigate, not a rule to obey. Some people may genuinely react; most probably don't; and only your own pattern can tell you which group you're in.

Why reactions are dose-dependent and delayed

Two features make tyramine especially hard to pin down by memory.

First, it's dose-dependent. The reaction tracks how much tyramine you actually consumed - and "aged cheese" is not one dose. Tyramine content rises with ageing time, ripeness, temperature and storage past freshness, so two servings of nominally the same food can deliver very different amounts.1 A small portion of a fresh batch may do nothing; a large portion of a well-aged one may be a different story.

Second, food-related migraine triggers are often delayed - an attack can surface hours after the meal, not minutes. That delay is what makes self-attribution so unreliable: the brain blames the most recent or most memorable food, not the one a day earlier. Put those together and you have a trigger that varies in dose, acts on a delay, and only matters for some people. No wonder it feels random.

Migraines rarely come from one trigger

There's a final reason "I had cheese and was fine" proves nothing. Most migraine attacks aren't caused by a single thing - they happen when several triggers stack within the same window and push an excitable brain past its threshold.

A tyramine-rich meal on a well-slept, unstressed, well-hydrated day may do nothing at all. The same meal after a short night, a skipped lunch, and around a hormonal shift can be the input that tips you over. So tyramine, where it matters, is usually one contributor to a stack - not a lone villain. This is also why blanket, permanent avoidance of every aged food is rarely the right answer: migraine triggers are individual, and the better approach is to identify your own rather than obey a generic list.3 Cutting everything on suspicion is restrictive, often unnecessary, and it never tells you whether tyramine was actually yours.

I'd banned cheese and cured meat for years on the standard advice. When I finally tracked it properly, tyramine-heavy meals only ever preceded an attack when they landed on a bad-sleep week. On a normal week, they did nothing. I got a lot of food back.

The goal: your triggers, and fewer attacks

Migraine is multifactorial to its core - genetics, sleep, stress, hormones, hydration and food all feed in. You can't change your genes, but the inputs you can influence are often enough to lift your threshold and bring attacks down in frequency.

That's the goal worth keeping in view: not cataloguing tyramine for its own sake, but fewer attacks. Tracking serves it in two ways - it works out whether tyramine genuinely belongs on your personal list, and it shows the trend: are your attacks becoming less frequent over the months? An attack-frequency trend is a clear, concrete thing to share with a doctor.

And keep the tracking light. A migraine brain doesn't need a demanding logging routine added to its load - a few seconds a day is enough. The aim is to understand your triggers and recover, not to monitor yourself relentlessly.
