Elimination diet vs low FODMAP: which should you try?

They're often presented as rival options. They aren't - one is a specialised version of the other.

You've decided food is part of the problem. You start reading - and two names come up again and again: the elimination diet and the low FODMAP diet. They're usually presented as a fork in the road, two competing plans, and you're meant to pick one.

That framing is slightly off, and clearing it up makes the choice far easier. So here's the honest comparison: what each one actually is, how they differ, and which one fits which kind of symptoms.

First: they're not really rivals

Here's the thing the "versus" framing hides. The low FODMAP diet is an elimination diet. It's a specific, highly structured, carbohydrate-focused version of one - listed alongside the gluten-free diet as a "highly regimented" elimination diet, distinct from broader, general elimination diets.1

So the real question isn't "elimination diet or low FODMAP." It's: do you need a broad, general elimination diet - one that can catch any food culprit - or the narrow, standardised carbohydrate-specific one? Both follow the same underlying logic: remove, let symptoms settle, reintroduce systematically. They differ in what they remove and how precisely they're protocolled.

Scope: broad net vs precise probe

The clearest difference is what comes off the plate.

A general elimination diet removes whole foods or food groups - very often the major allergens, since a handful of them account for the large majority of food allergies.2 It's a broad net: it can, in principle, catch any food culprit, including proteins, additives, and amines.

The low FODMAP diet doesn't remove "foods" in that sense at all - it removes a single class of carbohydrate. FODMAPs are short-chain carbohydrates that are poorly absorbed in the small intestine and fermented by gut bacteria.3 They cut across many foods - wheat, onion, garlic, certain fruits, dairy, legumes, some sweeteners - so the diet feels broad, but it's actually a precise probe aimed at one mechanism. That difference is a genuine trade-off. The general elimination diet's broad net can catch a trigger the low FODMAP diet would miss entirely - a true milk-protein allergy, an additive sensitivity, a reaction to amines. But the low FODMAP diet's precision means a much more standardised, validated protocol when the mechanism it targets is the one in play.

Mechanism: different problems

They're aimed at different things going wrong.

A general elimination diet targets food allergies, intolerances and hypersensitivity-type reactions - and it's used across a range of conditions, not just gut ones.1 The low FODMAP diet targets one specific thing: the osmotic and fermentation effects of carbohydrates in a gut with visceral hypersensitivity - the extra water and gas that, in a sensitive gut, drive the symptoms of IBS.3

So if your symptoms are the classic functional-gut picture - bloating, pain, wind, altered bowel habits - the low FODMAP mechanism is a strong match. If your symptoms range wider, or point at a protein or additive, the general elimination diet's broader scope fits better.

Evidence: low FODMAP is the better-studied one for IBS

For IBS specifically, the low FODMAP diet is the more rigorously evidenced of the two. Randomised controlled trials show it improves bloating, pain and stool consistency in IBS, and it's named in major gastroenterology guidelines.3 Clinical trials suggest up to around three-quarters of people with IBS see meaningful improvement within roughly six weeks of starting it.3

General elimination diets also have trial support - including for IBS and for migraine-type headaches1 - but because the protocol is empiric and individualised rather than one standardised diet, the evidence is broader and less concentrated. The short version: for textbook IBS, low FODMAP has the deepest evidence base; for a wider or non-gut picture, the general elimination diet is the more appropriate tool even if its evidence is more diffuse.

Effort: both are demanding

Don't choose based on hoping one is easy - neither is.

The low FODMAP diet, despite being "just carbohydrates," is complex enough that it's best run across all three phases with a trained dietitian; done from social-media advice, it tends to become overly restrictive or incomplete.3 A general elimination diet is demanding in a different way - strict avoidance, hidden-source vigilance, and a long sequence of one-at-a-time challenges. Which one suits you?

Here's the decision logic, in plain terms.

Lean toward the low FODMAP diet if your symptoms are gut-centred - bloating, pain, wind, diarrhoea or constipation - and you have diagnosed or strongly suspected IBS. It's also the supported choice for functional bloating and for IBS-type symptoms alongside conditions like SIBO.3 If your picture looks like textbook IBS, this is the evidence-led first move.

Lean toward a general elimination diet if your symptoms extend beyond the gut - skin flares, headaches, fatigue, aches - or if you suspect a specific food allergy or intolerance, or a non-FODMAP trigger such as an additive, an amine, or salicylates.4 It's also the logical next step if you've genuinely completed a low FODMAP diet and it didn't resolve things - that result itself suggests the trigger isn't a FODMAP.

A reasonable sequence for many people with gut-dominant symptoms: try the low FODMAP diet first, because it's the most validated and standardised; if it doesn't deliver, widen to a general elimination diet to catch what a carbohydrate-focused probe can't.

What they share

For all their differences, the two have the same skeleton - and the same rules.

Both are time-limited diagnostic tools, not lifestyles. Both depend entirely on the reintroduction phase - the strict phase only clears the noise; the challenges are where the answers are. And both are a poor fit - and a real risk - for anyone with a history of disordered eating, because each can reinforce restrictive patterns; that's worth honest reflection, and ideally a dietitian's involvement, before starting either.3

One more shared rule: before either, it's worth having reasonable grounds to think food is involved at all, rather than reaching for a restrictive diet on a hunch.

I'd been agonising over which "diet" to pick, as if they were opposing camps. Once I understood low FODMAP was just the structured carbohydrate version, the choice was obvious - my symptoms were textbook IBS, so I started there. It worked. I never needed the broader one.

The goal is the same either way

Whichever path you take, the destination is identical: not a permanently shorter food list, but the widest sustainable diet with only your genuine triggers limited. Both diets are experiments with an endpoint - restriction, then reintroduction, then a personalised, broad, livable way of eating.

That's recovery in practical terms. And both diets, tracked over weeks, draw a trend line of whether things are genuinely improving - the clearest answer to "is this working," and exactly the kind of evidence worth sharing with a doctor or dietitian.

Keep the tracking light, on either diet. They're both admin-heavy enough on their own - logging should take seconds, not become its own burden. The aim is to finish the experiment and get back to eating freely.
