Why does my stomach hurt after eating?
The cause is usually written in two clues - where it hurts, and when.
You finish a meal and the pain arrives - an ache, a cramp, a burn, or a heavy, uncomfortable fullness that won't shift.
Stomach pain after eating is one of the most common complaints there is. It's also not one thing - it's a handful of different causes that happen to share a symptom. The good news: you can usually narrow it down a long way using just two clues - where it hurts and when it starts.
The two clues that do most of the work
Before the causes, the framework. Pain after eating tends to sort itself by location and timing.

And timing adds the second layer: pain within minutes often points to overeating, gas, reflux or a sensitive gut, while pain that builds hours later points more toward food intolerances and how the meal is being fermented further down.
The common causes
Indigestion (dyspepsia). This is the umbrella term for the uncomfortable fullness, bloating and burning many people get shortly after eating.1 It's frequently brought on by overeating, eating too fast, or fatty and spicy meals - the everyday mechanical causes.
Functional dyspepsia. When that upper-abdominal pain and early fullness keep happening with no structural cause behind them, it's often functional dyspepsia - sometimes called a "nervous stomach." It can produce real pain and a feeling of fullness even after a modest meal, and the discomfort can last for hours.2 It's strongly tied to the gut-brain axis.
Gastritis. Inflammation of the stomach lining - from excess acid, H. pylori infection, or regular NSAID use - which can cause a gnawing or burning upper-abdominal pain after eating.3
Acid reflux. When stomach acid flows back up, it produces a burning pain that can sit high in the abdomen or behind the breastbone, often worse after large, fatty or acidic meals.
IBS. Here the pain is typically lower and crampy, it's linked to your bowel habits, and - a key distinguishing feature - it's often eased by passing stool.4
Gallbladder problems. Pain under the right ribs that arrives 15-20 minutes after a fatty meal points toward the gallbladder.
Food intolerances. FODMAPs, lactose and others can cause crampy pain, gas and bloating - and this kind is often delayed, building hours after the meal as the food is fermented in the gut.
Functional dyspepsia or IBS? A useful distinction
These two get confused constantly, because both are common and both involve gut-brain signalling. The simplest way to tell them apart:
- Functional dyspepsia sits in the upper abdomen, is centred on eating, and is not relieved by going to the toilet.2
- IBS pain is lower, is tied to bowel habits, and often is relieved by passing stool.4
They can also coexist - plenty of people have both - but knowing which pattern you mostly have tells you a lot.
Why it's rarely just one thing
Stomach pain after eating is genuinely multifactorial. The food is part of it - but so is how much and how fast you ate, how stressed you were, how well you slept, and how sensitive your gut is that day.
The gut-brain axis runs through almost all of this: stress and anxiety measurably change how the gut moves and how strongly it registers sensation, which is why the same meal can be painless on a calm day and painful on a frantic one.5 You're rarely reacting to a single input - you're reacting to the combination.
How to find your pattern
Because the causes sort by location, timing and what eases them, those are exactly the things to track:
- Where the pain sits - upper-central, lower, or under the right ribs.
- When it starts - minutes, or hours later.
- What eases it - passing stool, or nothing.
- Which foods, how much, how fast - and the context: stress, sleep, the rush.
Across a few weeks, those notes turn a vague "my stomach hurts after eating" into something specific - "crampy lower pain a few hours after high-FODMAP lunches" or "upper fullness after large, fast meals on stressful days."
